Commitment to Privacy

Simplifeye Optical is committed to protecting the privacy and security of personal health information in accordance with the Personal Health Information Protection Act (PHIPA) of Ontario.

Personal health information may include information about your vision, prescriptions, eye health history, insurance or funding details, and contact information.

Collection of Personal Health Information

​​

Simplifeye Optical collects personal health information only as necessary to:

  • coordinate eye care services and Community Vision Clinic appointments

  • assist with eyewear consultations and dispensing

  • communicate regarding appointments and services

  • process insurance or funding claims when authorized

  • maintain records necessary for continuity of care

 

Information is typically collected directly from clients through intake forms, consultations, and clinic visits.

Community Vision Clinic Collaboration

​​

Simplifeye Optical collaborates with independent optometrists to deliver Community Vision Clinics in senior residences, community facilities, and other care settings.

​

Each optometrist participating in the clinic remains responsible for the clinical services they provide and for maintaining the medical record associated with the eye examination.

​

Simplifeye Optical supports the coordination of services, scheduling, and eyewear dispensing and may access relevant information as necessary to provide these services.

​

Eyewear Services

​

Simplifeye Optical also provides independent eyewear consultations and dispensing services in community settings and private appointments.

​

For eyewear-only services, Simplifeye Optical acts as the custodian of the personal health information collected for the purpose of providing eyewear recommendations, managing prescriptions, coordinating orders, and maintaining appropriate client records.

Electronic Medical Records

​​

Personal health information may be stored in Halaxy, a secure electronic medical record system used to coordinate care within the Community Vision Clinic program.

​​

Simplifeye Optical hosts the secure electronic medical record platform used for coordination of services. Participating optometrists are granted access to the system for the purpose of documenting and maintaining clinical records related to the care they provide.

​​

Halaxy uses encrypted, access-controlled cloud infrastructure designed to support healthcare privacy and security standards consistent with PHIPA requirements.

​​

In some cases, information may be securely stored on servers located outside Canada.

​​

Safeguards

​​

Simplifeye Optical takes reasonable steps to protect personal health information including:

  • secure electronic medical record systems with encryption

  • password-protected access controls

  • role-based access permissions

  • secure backups and monitoring

  • limiting access to authorized individuals only

 

If a privacy breach occurs that may affect your personal health information, Simplifeye Optical will notify affected individuals and follow applicable reporting requirements under PHIPA.​​

Use and Disclosure

​​

Personal health information is used only for purposes related to providing eye care services and supporting client care.

​

Information may be shared with:

​

  • the optometrist providing your clinical care

  • laboratories or suppliers involved in producing eyewear

  • insurers or funding programs when authorized

  • other regulated healthcare professionals involved in your care when appropriate

​

Simplifeye Optical does not sell personal information.

​​

Marketing and Communication Preferences

​

Simplifeye Optical may occasionally send newsletters, service updates, or educational information related to eye care or eyewear services.

​

These communications are sent only to individuals who have explicitly opted in to receive them. You may withdraw consent and unsubscribe from marketing communications at any time by using the unsubscribe link included in the message or by contacting us directly.

​

Personal health information is not used for marketing purposes without consent.

​

Access and Correction

​​

Clients have the right to:

  • request access to their personal health information

  • request corrections to inaccurate information

  • withdraw consent for certain uses of their information

​
